All new patients must fill out the Patient Signature Form. If the new patient is an adult, also complete the Patient Information form and the Patient Confidential Information form. If the new patient is a child or adolescent, also complete the Child/Adolescent Patient Information form and the Confidential Patient Information – Child and Adolescent form. Bring the completed forms with you at your initial appointment. If after establishing an appointment it becomes necessary for you to cancel the appointment we would appreciate a twenty-four (24) notice if at all possible.
